Share Your Experience, Shape the Community
What do you think can strengthen the health of your community? Now is the time to share your experiences in order to see change implemented in Central Vermont.
Every three years, residents of Washington County and surrounding areas are encouraged to join the conversation about how to strengthen community health. More than 50 local organizations, including county health departments, local universities, health care providers, community-driven non-profits, and religious organizations, are working together to promote a Community Health Needs Assessment (CHNA).
This year’s CHNA survey results will inform a three-year Community Health Improvement Plan that:
- Identifies and prioritizes health needs
- Builds upon existing strengths
- Works on solutions with community partners
