Care Coordination

As our lives grow richer with age, we may also encounter challenges to our independence. CVCOA’s case management team works with older Vermonters who need ongoing support, and their families and caregivers, to address the challenges that may prevent them from living independently.

Our case managers support older Vermonters to identify areas where extra help is needed.

Case managers partner with the older person and their family to develop a comprehensive “personalized care plan” to provide the supports needed to remain in their home and community. Case managers will access the benefit programs and coordinate the care needed to implement the personalized care plan.

Long-Term Care

Frail or disabled adults who medically and financially qualify for nursing home level of care may be eligible for Choices for Care, Vermont’s Long-Term Care Medicaid Program. This program provides individuals with a choice of settings in which to receive care: a nursing home, residential care home or in-home support. In-home supports include case management, personal care (e.g., help with bathing or dressing), respite care, adult day health center services, assistive devices and home modification. Choices for Care - Moderate Needs can provide assistance with adult day and some homemaker services. Veteran-Directed Care Program

Veteran-Directed Care (VDC) provides veterans of all ages who require help with activities of daily living—for example, bathing and dressing, medication, and financial management—to receive services and support in their home.
